Systemic Capillary Leak Syndrome as a Rare, Potentially Fatal Complication of COVID-19: A Case Report and Literature Review

Systemic capillary leak syndrome (SCLS), also known as Clarkson’s disease, is a rare and potentially lethal condition characterized by hypotension, hemoconcentration, hypoalbuminemia; however, the cause of SCLS still uncertain. We present case 62-year-old male with flu-like symptoms who presented to emergency department shock. Initial evaluation revealed hypoalbuminemia, acute kidney failure, positive polymerase chain reaction (PCR) for severe respiratory coronavirus 2 (SARS-CoV-2). Despite aggressive fluid resuscitation, shock persisted, patient’s deteriorated. After ruling out ischemia septic shock, patient was diagnosed disease 2019 (COVID-19)-associated SCLS. Treatment remdesivir intravenous immunoglobulin (IVIG), along restoration intravascular volume, led gradual improvement condition. The experienced pulmonary edema, which managed correcting balance through continuous hemodiafiltration. Eventually, recovered without any residual organ complications. often misdiagnosed because its rarity non-specific symptoms. Accurate diagnosis understanding disease’s pathophysiology are crucial effective management. This report contributes existing literature presenting COVID-19-associated emphasizes need further research on occurrence outcomes.
